    REGISTRATION FORM
    An invoice will be sent to you upon receipt of your registration.
    Please allow two business days after registration to receive your invoice.
    Invoices will come from a “squareup” address; our payment processor.
    ​We accept Visa, Mastercard, district P-cards,  personal checks, district checks.
    TMT 10-WEEK COURSE DEPOSIT

    $50.00 non-refundable Registration Fee – due to hold your place in the cohort (first come first served).
    Remainder of tuition ($340.00) due 2 weeks prior to start date.

    REFUND POLICY

    ​20% of each course, tutorial or workshop fee is non-refundable to cover processing.

    TMT 10-WEEK COURSE:
    • 100% of the remaining refundable tuition is refundable up until midnight of the Tuesday before the class is scheduled to begin.
    • 50% of the tuition is refundable until midnight of the second Tuesday of the course (the end of Week 1).
    INDEPENDENT TUTORIALS
    • 100% of remaining refundable fee for tutorials, for 24 hours after purchase. All materials must be returned, and a non-disclosure affidavit submitted.
    • If you haven’t completed a course or tutorial within six months of your start date, your account will be closed out.
    MASTER WORKSHOP
    • Remaining 100% of the fee is refundable up until 48 hours before the start time of the workshop.
    • Icy or other extreme weather conditions may result in rescheduling of workshop, at presenter’s discretion. No refund will be issued if rescheduled, and above conditions apply.
    • A list of equipment will be provided to the teacher, for set-up prior to presenter's arrival.
    • Upon receipt of this registration form, teacher will be contacted to schedule date and time of workshop.
